Message to your own family doctor

If you have called upon the assistance  of The After Hours General Practitioners Clinic , then your own family doctor will always  receive a report about your contact with The After Hours General Practitioners Clinic  on  the next working day. This concerns the complaint, the advice and/or the  treatment. Further, dates and times, and those with whom you have been in contact  will be mentioned in these reports. In case  of an advice, the name of the doctor on duty who has checked the advise will also  be mentioned.

 

 

Your details
Any information that you pass on during the telephone conversation will be stored in a dossier at. The After Hours General Practitioners Clinic. All medical information
is covered by medical professional confidentiality act, which is accessible only to those who take part in your treatment. Your administrative details, such as address and insurance information, will be treated with the same confidentiality and serve in order to correspond with you or with your health insurance company.
In case you have had contact with The After Hours General Practitioners Clinic before, your earlier contact details will also be viewed and, when necessary, be used in your treatment process.
The general practitioner who treats you at The After Hours General Practitioners Clinic, replaces your own family doctor. This G.P. will therefore also use, wherever possible, your data kept in the dossier of your own family doctor. This is not yet possible for  all patients.


Your data will be confidentially treated, and is covered by medical professional confidentiality act.

 

Perusal

The assistant on the phone may not always be able to look into the files of the own doctor, hospital or other therapists. Also, she can not always see your medication details. It is therefore very important that you report this information to the assistant when asked, or you consider it relevant to mention during your phone call.
